    Periodontal disease

    Periodontal disease, also known as gum and supporting structure disease, is often an unnoticeable condition affecting the tissues surrounding and supporting the teeth. It is caused by the accumulation of plaque, a sticky layer of bacteria that constantly forms on the teeth. If the plaque is not removed by regular brushing and flossing, bacteria from it can lead to inflammation of the gums, i.e. gingivitis. If gingivitis is not treated, it can turn into periodontitis, where the inflammation extends deeper into the supporting structures of the teeth, including the bone, which ultimately leads to loose teeth, their falling out or the need for extractions at the dentist.

    Gingivitis

    Gingivitis is a common and mild form of periodontal disease. Some of the common signs and symptoms of gingivitis:

    1.  bleeding gums during and after brushing or flossing
    2. red, swollen or sensitive gums
    3. persistent bad breath (halitosis)
    4. receding gums

    Gingivitis is primarily caused by bad oral habits that lead to the accumulation of plaque at the level of the gums. In this random stage of the disease, the damage can be repaired since the bone and connective tissue are not yet affected. Good oral hygiene is essential for the prevention and treatment of gingivitis.

    Periodontitis

    Periodontitis is an advanced stage of gum disease that extends to deeper bone structures and supporting tissues. Some of the key features and characteristics of periodontitis:

    1. receding gums
    2. persistent bad breath
    3. creating deep pockets between teeth and gums
    4. a change in the arrangement or spacing of the teeth
    5. bone loss

    Periodontitis is preceded by gingivitis, but not everyone with gingivitis progresses to periodontitis. At this stage, the supporting bone and fibres that hold the teeth in place are irreversibly damaged. The gums begin to form pockets below the level of the gums, where plaque and food accumulate, leading to the destruction of the gums and bone. As the disease progresses, the pockets deepen and more and more gum and bone are destroyed, ultimately leading to tooth loss. Poor oral hygiene, smoking, genetic factors, systemic diseases and certain medications can increase the risk of developing periodontitis.

    Process of periodontal therapy

    The goal of periodontal disease treatment is to control the existing infection and stop the progression of the disease. The early stage, i.e. gingivitis, can be cured with proper oral hygiene without consequences. A more advanced stage, i.e. periodontitis, requires additional procedures. The choice of procedure depends on the progress of the disease, on the response to previous therapy or on general health. Treatment duration, degree of discomfort and healing time vary. The choice of therapy includes home care, non-surgical, antibiotic and surgical therapy.

    1. Home care

    It includes a healthy diet and proper brushing and flossing; brushing removes plaque from the surface of the teeth, while flossing removes plaque between the teeth and below the gum level

    2. Non-surgical therapy

    This includes professional cleaning, i.e. removing tartar above and below the level of the gums. If bone loss or receding gums are observed, standard therapy includes root scraping and polishing. This method removes plaque and calculus and smoothes the rough surface of the root to prevent further bacterial accumulation. This process helps the gums reattach to the teeth.

    3. Antibiotic therapy

    Antibiotics can be prescribed along with some of the therapies or independently to reduce or temporarily eliminate the bacteria associated with periodontal disease. They can be given in different forms, as oral antibiotics or topical antibiotic treatments such as gels or mouthwashes.

    4. Surgical therapy

    In advanced cases of periodontal disease, surgical procedures may be necessary.

    Some of the common procedures are:

    1. Flap lift surgery includes raising the gums and removing tartar. After that, the gum is sutured in such a way that it is tightly attached to the teeth. This method reduces the depth of pockets and areas where bacteria grow.
    2. Bone grafting or bone grafts are used in cases of significant bone loss to replace the lost bone and promote tissue regeneration.
    3. Soft tissue transplantation or soft tissue grafts involves taking tissue from one part of the oral cavity and placing it in the area where the gums have receded.
